Employee Probationary Contract

I,
<name>, understand that I am a probationary employee until the following
conditions are met within six months of the date of hire (deadline date of
<date> based upon a hire date of <date>):
q I have taken and
satisfactorily completed (missed no classes and accomplished the class goals) a
basic obedience class from a Canine Club instructor;
q I have read and understand The Culture Clash by Jean Donaldson;
q I have read and understand All About Dog Daycare by Robin K.
Bennett;
q I have read and understand How to be Leader of the Pack... by Patricia McConnell;
q I have had satisfactory
performance evaluations; and,
q I have received the stamp of
approval from the owners.
I
can get off of probation earlier than six months by meeting the above
conditions.
If
I fail to meet all of the above requirements by the deadline, I will be
terminated.
My
probationary pay is $7.00 per hour. If I
successfully pass my probationary period, I will receive $8.00 per hour. I can expect an annual cost of living
increase on my anniversary dates, if my performance has been continually
satisfactory to date and if the business’ financial condition can accommodate
the increase. I will have to earn merit
pay raises; these can include, but are not limited to: being reliable (at work
on time, working when scheduled, etc.), continuing education, more than
satisfactory performance on a continuing basis, taking on additional responsibilities,
and going above and beyond expectations.
Benefits I will receive after probation include, but are not
limited to: free classes, seminars/conferences, and other events hosted by The
Canine Club and at-cost supplies from The Canine Club store. After one month of satisfactory employment, I
will also receive free dog day care, if my dog is eligible for it (I will not
bring my dog to day care until eligible).

“Dog Day Care Attendant Job Description” is included by reference.
I
